ASUM-CCPU Supervisor requirement for enrolment
Candidates are welcome to complete the Primary Supervisor Declaration Form at any time following enrolment and upload it to their profile in ASUM Learn. Please note, however, that the supervisor listed on this form must match the supervisor recorded in your logbook. This is essential, as all documentation will be reviewed together prior to sign-off being granted for the relevant course.
To avoid any delays in the sign-off process, we recommend double-checking that the Nominated Primary Supervisor meets the supervisor criteria on the ASUM-CCPU regulations and that paperwork reflect the same supervisor details before submission.

E-learning Physics
Please enrol in the e-learning physics when you complete your enrolment form in addition to the ASUM-CCPU units.

Completing the physics module is compulsory to complete the ASUM-CCPU.
ASUM-CCPU CAHPU Accredited course provider
Accredited courses can be completed at any time during the 3 years of enrolment. Validity of the course is 2 years from when it was attended.
Logbooks can be completed before or after attending an accredited course. It is the Supervisor’s responsibility to determine when the candidate is ready to commence the logbook.
ASUM-CCPU Recertification grace period
ASUM-CCPU holders are required to recertify at three-yearly intervals after the initial certification date recorded on the issued ASUM-CCPU certificate.
A One-year grace period is determined to complete the recertification process. In the case of an expired ASUM-CCPU (3 years after the original recertification due date), the applicant will need to complete the return to practice process to get recertified.
Logbook numbers must not include those from the accredited training course.
